THE BACK IN ACTION METHOD™ / COACHING APPLICATION

Your story.
Your next chapter.

Tell me a little about where you are now and what you’d like to get back to. Your answers help start a conversation about whether coaching is a good fit.

Applications are open to adults aged 18 and over in Australia and internationally. Suitability and service availability are confirmed before enrolment.

32 questions, in five short sections. Review your answers before sending. Applying does not commit you to a programme.

This form is not monitored for emergencies and does not provide medical clearance. Safety concerns take priority over coaching or product offers. No symptoms reported does not rule out a serious condition.

Fields marked * are required. Your answers stay in this page until you submit; they are not saved if you close or reload it.

YOUR INFORMATION

A little care with your answers.

This application stores your contact details, answers, assessment and review history in Joseph’s coaching CRM and sends your application to hello@josephbitar.com.au for review and follow-up. Please include only information you are comfortable sending by email; do not include medical records or identification documents.

When sending is enabled, Cloudflare processes the website request and anti-spam check, Resend processes email delivery, and Microsoft 365 receives the application in Joseph’s inbox. Your answers may include health information. CRM records are stored in the configured database (local SQLite in development; Cloudflare D1 when deployed). Submitted emails remain with the recipient and service providers under their applicable retention practices. This page does not save a draft in your browser storage or subscribe you to marketing. To ask about your information, request access or a correction, or discuss deletion, contact hello@josephbitar.com.au.
